Ikea Again Announces Dresser Recall After Death of 8th Child

Ikea has relaunched the recall of what had been 29 million chests and dressers following the death of an eighth child.

CEO Lars Petersson said the company wants to increase awareness of the recall for several types of chest and dressers that can easily tip over if not anchored to a wall.

The death of a California toddler, who was found trapped underneath an Ikea Malm dresser in May, has raised questions about whether Ikea has effectively spread the word about the recall, which was first announced in June 2016.

Petersson said Ikea has had an "extensive communication" campaign through social media and its website. The Swedish company emailed 13 million people about the recall two months ago, he said.

In December, Ikea settled wrongful death lawsuits for $50 million with three families of children killed by tipped-over dressers.

At least eight children under the age of three have been killed when an Ikea dresser fell on them, according to the Consumer Product Safety Commission. The first death occurred 28 years ago and the others occurred after 2002.

The latest death was Jozef Dudek, 2, of Buena Park, California. The toddler had been put down for a nap when his father went in to check on him and found him under the dresser, according to details released by lawyers retained by the family.

Alan Feldman, one of the lawyers representing the Dudek family, said Ikea's recall has been "poorly publicized" and "ineffective in getting these defective and unstable dressers out of children's bedrooms."

The same team of lawyers represented the families of the three toddlers whose death resulted in the $50 million settlement.

Petersson said that more than 1 million dressers of have been returned for a refund or have been secured to walls with Ikea's help since 2015, when the company first offered free wall-mounting kits.

About 17.3 million units are affected by the recall, according to the notice sent out by the U.S. Consumer Product Safety Commission Tuesday. The agency said the number of units in the recall was revised down by Ikea from June 2016.

They should be anchored to the wall or placed in an area kids can't get to. Ikea is providing refunds or free wall-anchoring kids to people who have the product.

Stay Away From Romaine Lettuce, Consumer Reports Advises

A new warning has been issued by Consumer Reports to avoid romaine lettuce while U.S. and Canadian health officials continue their investigation after 58 people were reported sick from E. coli infections, NBC News reported. One person has died.

The Food and Drug Administration and Centers for Disease Control and Prevention have issued warnings about the outbreak. 

The outbreak spread across 13 states: California, Connecticut, Illinois, Indiana, Michigan, Nebraska, New Hampshire, New York, Ohio, Pennsylvania, Virginia, Vermont and Washington. The Public Health Agency of Canada reported on 41 illnesses, NBC News reported. 

According to Consumer Reports, this strain of E. coli has a toxin that could lead to serious illness, kidney failure and even death in some cases.

NBC reported that it could take weeks to to track down the source of an outbreak because most food is shipped to central locations from various farms, where it is processed, packaged and redistributed.

Takata Adds 3.3M Air Bag Inflators to Massive Recall

Japanese air bag maker Takata is recalling an additional 3.3 million faulty air bag inflators as it expands the largest automotive recall in U.S. history.

The latest recalls cover frontal air bags in certain 2009, 2010 and 2013 vehicles made Honda, Toyota, Audi, BMW, Daimler Vans, Fiat Chrysler, Ford, General Motors, Jaguar-Land Rover, Mazda, Mercedes-Benz, Mitsubishi, Nissan, Subaru and Tesla. Automakers will provide specific models in paperwork that will be filed later this month with the National Highway Traffic Safety Administration.

Notices of the expanded recalls were posted Saturday on the agency's website.

Takata uses the chemical ammonium nitrate to create a small explosion and fill air bags quickly in a crash. But the chemical can deteriorate when exposed to high humidity and temperatures and burn too fast, blowing apart a metal canister. That can hurl hot shrapnel into unsuspecting drivers and passengers.

At least 20 people have been killed worldwide and more than 180 injured.

The latest recalls are part of the largest series of automotive recalls in U.S. history, with 19 automakers having to recall up to 69 million inflators in 42 million vehicles. The problem brought a criminal conviction and fine against Takata and forced the Japanese company into bankruptcy protection.

The recalls, which are being managed by NHTSA, are being phased in over the next three years. Older models and those in states with high humidity and temperatures are getting priority.

Many automakers have been slow to replace the potentially deadly inflators. A report by an independent monitor said that as of Sept. 15, 2017, automakers have recalled 43.1 million inflators. Of those, only 18.5 million, or 43 percent, have been replaced even though Takata recalls began in 2001.

NHTSA has said the Takata recalls are unprecedented in size and complexity and have resulted in groundbreaking lessons that will help automakers reach their repair goals. The agency said it is monitoring the automakers' progress and working to expand best practices to boost completion rates. The agency also has the authority to fine automakers that don't make recall repairs in a timely manner.

Toyota Adds 601,000 Vehicles to Takata Air Bag Recalls

Toyota is recalling 601,000 more vehicles in the U.S. to replace defective and potentially deadly Takata air bag inflators.

The company says the recall covers the Toyota Corolla and Matrix, Scion xB, Lexus IS250 and 350, and the Lexus IS-F from 2009, 2010 and 2013. Also covered are the 2010 and 2013 Toyota 4-Runner, Lexus IS250C and 350C and Lexus GX460, as well as the 2009 and 2010 Toyota Yaris and Lexus ES350, and the 2013 Toyota Sienna.

The recalls come after Takata revealed that another 3.3 million inflators are defective. Its inflators can explode with too much force and hurl shrapnel. At least 20 people have died worldwide and more than 280 have been hurt.

Owners will be notified by March and dealers will replace front passenger inflators or air bag assemblies.

About 7,900 High Chairs in US Recalled Due to Fall Hazard

Skip Hop is recalling about 7,900 high chairs in the U.S. after receiving some reports of the front legs of the chairs detaching, causing a fall hazard for young children.

Of the 13 reports received about the issue, two children have been bruised from the faulty high chairs, according to Skip Hop.

The recall is for the gray high chair that can be converted into a toddler chair. The style numbers are 304200 and 304200CN, with any of the following date codes on the back of the chair: HH102016, HH11/2016, HH3/2017 and HH4/2017.

The company also is recalling about 2,000 of the chairs in Canada.

The company suggests that consumers stop using the impacted models immediately.

Fiat Chrysler Recalls Minivans for Engine Stalling Problem

Fiat Chrysler is recalling more than 160,000 minivans mainly in North America to fix a software problem that can make engines stall.

The recall covers certain Chrysler Pacifica vans from the 2017 model year with gasoline engines.

The company says under rare conditions, the engine control computer can incorrectly determine the operating status and cause the engine to shut off. Fiat Chrysler says it knows of one potentially related crash but no injuries caused by the problem.

Dealers will upgrade engine control software. Customers will be advised when to bring their vans in for the free service, which will be available sometime before April 1.

The recall comes about two months after the a Virginia family worked with nonprofit Center for Auto Safety and filed a petition asking the government to seek a recall of the vans.

Allison Cohen, of Leesburg, Virginia, told NBC Washington her car stalled in the middle of the intersection in her neighborhood. Cohen was one of over 50 Pacifica minivan owners who filed a complaint to the National Highway Traffic Safety Administration.

Fujifilm Recalls Hundreds of Thousands of Wall Plugs Over Shock Concerns

A major New York-based camera maker is recalling nearly 300,000 power adapter wall plugs sold with digital cameras nationwide over concerns about a potential shock hazard, the U.S. Consumer Product Safety Commission said Tuesday.

Fujifilm, headquartered in Valhalla, issued the voluntary recall for 270,000 plugs sold in the U.S., along with 24,000 were sold in Canada, because the plugs can crack, break or detach and get stuck in the wall, the company said in a statement. They can also expose live electrical contacts, posing a shock hazard.

No injuries were reported in connection with the recalled products, which were sold with digital cameras in-store at retailers across the country and online at Amazon.com and other websites. The products were made in China. 

Specifically, the recall involves AC-5VF power adapter wall plugs sold with Fujifilm digital camera models XP90, XP95, XP120, XP125, X-A3 and X-A10. The digital cameras were sold in a variety of colors. The recalled wall plugs are black and are combined with a power adapter and USB cord that plugs into the adapter. Model number "AC-5VF" is printed on the back of the power adapter. The serial number is printed on the bottom of the camera or under the battery compartment lid. The XP90 and XP95 were sold from June 2016 through January 2018, the XP120 and XP125 were sold from January 2017 through January 2018, the X-A3 was sold from October 2016 through January 2018, and the X-A10 was sold from February 2017 through January 2018. The digital cameras cost between $160 and $600 with the power adapter wall plugs.

Anyone who has a recalled power adapter wall plug should stop using it immediately and contact Fujifilm for a free replacement. Panera Bread Recalls Cream Cheeses Over Listeria Concern

Panera Bread Co. is voluntarily recalling some cream cheese sold in its U.S. stores because of possible bacterial contamination.

The St. Louis-based company released a statement Sunday saying the recall was initiated after testing from a single production day indicated the presence of Listeria monocytogenes in one variety of its 2 oz. cream cheese.

Tests on cream cheese samples manufactured before and after that production run have all come back negative.

Panera says there have been no reported illnesses. The company says the recall is pre-emptive.

The products included in the recall are all varieties of unexpired 2 oz. and 8 oz. cream cheese products with an expiration date on or before April 2. The recall doesn't affect Panera Bread Canadian bakery cafes or any other products.

Toyota Recalls 645,000 Cars, Says Air Bags May Not Inflate

Toyota is recalling about 645,000 vehicles worldwide to fix an electrical problem that could stop air bags from inflating in a crash.

The recall covers certain Toyota Prius and Lexus RX and NX SUVs. Also covered are some Toyota Alphard, Vellfire, Sienta, Noah, Voxy, Esquire, Probox, Succeed, Corolla, Highlander, Levin and Hilux models. All were produced from May of 2015 to March of 2016.

The automaker says an open electrical circuit could occur over time. That would set off an air bag warning light and could stop the side and front air bags from deploying.

Dealers will inspect serial numbers on sensors and replace them if necessary at no cost to owners. Toyota will notify owners by letter in starting in late March.

Mitsubishi Recalls SUV and Car Models; Belt Can Come Loose

Mitsubishi is recalling about 640,000 cars and SUVs worldwide because the accessory drive belt can come loose and cause engines to stall.

The recall covers Outlander SUVs from 2008 through 2012, Outlander Sport SUVs from 2011 through 2012, Lancer cars from 2009 through 2012 and Lancer Sportbacks from 2010 through 2012. All have Mitsubishi's 4B11 or 4B12 engines, according to a document posted Thursday by the U.S. National Highway Traffic Safety Administration.

The automaker says a flange that automatically adjusts the belt tension can crack, causing the belt to detach. That will drain the battery and cause the engine to stall. Mitsubishi says it has no reports of crashes or injuries.

Dealers will replace the belt tensioner with an improved one at no cost to owners. The recall is to begin in the U.S. on March 27.

Dog Food Withdrawn Over Concerns About Euthanasia Drug

The J.M. Smucker Co. is withdrawing some shipments of dog food amid reports that it could be tainted with traces of a drug used to euthanize animals.

The company said Thursday it is pulling back shipments of several varieties of wet canned Gravy Train, Kibble 'N Bits, Skippy and Ol' Roy brands. It said it is investigating how the euthanasia drug pentobarbital got into its supply chain and is focusing on a single supplier of a minor ingredient used at one manufacturing facility.

The recalls come after WJLA-TV in Washington, D.C., said it tested 15 cans of Gravy Train. It found nine cans, or 60 percent of the sample, tested positive for pentobarbital.

Smucker cited experts noting that the low levels of the drug cited in the report do not pose a threat to pets.

"However, the presence of this substance at any level is not acceptable to us and not up to our quality standards," said the company, which is based in Orrville, Ohio. It said it does not use meat from euthanized animals in its pet food.

Instant Pot Company Recalls Melting Multicookers

If your Instant Pot is too hot it might be time to return what you've got. 

An Instant Pot company located in Ontario, Canada announced that it received a "small number" of Gem 65 8-1 Multicooker pots that have melted as a result of overheating. 

The company has asked consumers to check the code at bottom of their pots to see if they will be affected.  

People with the codes 1728, 1730, 1731, 1734 and 1746 should immediately stop using the product and call the company at 1-800-828-7280 Ext. 2. with any questions.

"We take any problem with our products extremely seriously as safety and quality are our primary concern, and we are working cooperatively with the US Consumer Product Safety Commission (CPSC)" the company said on Facebook.

Toyota, Hyundai Recall Roughly 110,000 Vehicles

Toyota and Hyundai have recalled roughly a combined 110,000 trucks and sport utility vehicles, including the 2018 Toyota Tundra and Sequoia, and the 2018 Hyundai Santa Fe and Santa Fe Sport.

In its recall announced Saturday, Hyundai says some of Santa Fe vehicles are at risk for the steering wheel breaking away from the steering column. Roughly 43,900 vehicles are included in Hyundai's recall.

Toyota recalled roughly 64,900 Tundras and Sequoias, saying these vehicles are at risk for having their electronic stability control systems shutting down unexpectedly.

Both car makers will notify impacted owners of the recalled vehicles starting in March, and if necessary, will repair the vehicles at no cost.

High Chairs Sold at Walmart Recalled Over Fall Hazard

About 36,000 Graco Table2Table 6-in-1 high chairs sold at Walmart have been recalled after at least five children were injured when their chair fell over.

The rear legs of the chair can pivot out of position, making it unstable, according to a recall notice posted last week.

The chairs were sold at Walmart stores from October 2016 to December 2017. They have the model number 1969721, which can be found underneath the toddler seat and on a label behind the booster seat.

Target Recalls Girls Studded Jeans Due to Laceration Hazard

Target has issued a recall on a pair of girls' skinny jeans due to reports of skin cuts and lacerations.

The Cat & Jack Girls' Star Studded Jeans from apparel manufacture Topson Downs were sold exclusively at the discount retailer's stores nationwide and online between September 2017 and November 2017 for $18.

The jeans were sold in sizes 4 to 18P and have metal stars on the front bottom of the legs. The words “Super Skinny” are printed inside the waistband of the recalled jeans and the attached hangtag has one of the following numbers at the top: 205-03-1377 to 205-03-1392.

According to the Consumer Product Safety Commission, Target has received six reports of the metal stars embellished around the bottom of the pants detaching and causing lacerations. Target is recalling 30,000 pairs of jeans.

Customers are advised to immediately stop wearing the recalled jeans and return them to any Target store nationwide for a full refund.

Ford Recalls Almost 1.4M Cars; Steering Wheel Can Come Loose

Ford is recalling nearly 1.4 million midsize cars in North America because the steering wheel can detach from the steering column and drivers could lose control.

The recall covers certain Ford Fusion and Lincoln MKZ cars from the 2014 through 2018 model years.

Ford says steering wheel bolts can loosen over time. The company says it knows of two crashes and one injury caused by the problem.

Dealers will replace the bolts with longer ones that have more aggressive threads and a nylon patch to stop them from coming loose.

Just over 1.3 million cars in the U.S. are being recalled. The rest are in Canada and Mexico.

Animal-Shaped Pacifier and Teether Holders Recalled

More than half a million animal-shaped pacifier and teether holders are being recalled because the snap that attaches to pacifiers and teethers can fray and detach, posing a choking hazard.

Sixty-seven reports of the ribbon on Dr. Browns Lovey pacifier and teether holders fraying and the snap detaching have been reported to Handi-Craft Company, which distributed the toys in the U.S. to retailers like Walmart Toys R Us, Babies R Us and others. There have been no reported injuries.

About 590,000 of the recalled products were sold in the U.S. between August 2015 and this month for about $10, according to the U.S. Consumer Product Safety Commission, which issued the recall notice.

Anyone with one of the holders (identifying information below) should take them away from children and contact the company for instructions on receiving a replacement or comparable merchandise. Kidde Recalls Around 450K Smoke Detectors Sold in US and Canada

About 452,000 smoke detectors sold at Home Depot, Walmart and other stores nationwide are being recalled after the company discovered the device may fail to alert consumers of smoke, the company said Wednesday.

The Kidde company, which sold the detectors in the U.S. and Canada, was recalling models PI2010 and PI9010 after a yellow cap was left on some alarms, possibly preventing the device from working. The yellow cap covers one of two smoke sensors, the company said. If the sensors are affected, homeowners may not be alerted to a fire.

The model and date are located on the back of the alarm.

  

Consumers were advised to remove the alarm to look for the presence of a yellow cap, and if the alarm has one, to contact Kidde customer service to get instructions on ordering a new one. You can call 833-551-7739 from 8:30 a.m. to 5 p.m. ET Monday through Friday, from 9 a.m.to 3 p.m. on Saturday and Sunday.

There has been one report of a faulty alarm, the company said. No injuries related to the recalled products have been reported.

The potentially defective alarms were sold September 2016 through January 2018, the company said.

Stella Artois Recalls Some Bottles Over Glass Particle Fears

Anyone with bottles of Stella Artois at home may want to check the labels — the beer company is recalling some packs of 11.2-ounce bottles because they may contain glass particles.

Stella Artois announced the voluntary recall Monday, saying a packaging flaw that originated with a third-party producer could cause a small piece of glass to fall into the beer. Anyone with beer that might be affected is eligible to be repaid, and the beer should not be consumed, according to the company.

The recall applies to Stella Artois 6-, 12-, 18- and 24-packs, "Best of Belgium" multi-packs and Stella Artois Légère 6- and 12-packs. Less than 1 percent of the beer's glass bottles sold in North America per year are affected, the company says.

"The safety of our consumers is our top priority. While the number of potentially-affected glass bottles is very small, we are recalling these Stella Artois packages as a precautionary measure," Stella Artois spokeswoman Christina Choi said in a statement. FDA Issues Mandatory Recall of Some Kratom Products

The Food and Drug Administration ordered the mandatory recall Tuesday of kratom products distributed by one company that may be contaminated with salmonella.

The FDA said it’s the first time it has used its mandatory recall power for a food product after first trying to get the company to voluntarily take the products back. The agency has been criticized for years by consumer advocates and some members of Congress who say it moves too slowly to recall potentially contaminated foods, NBC News reported.

Tuesday’s mandatory recall affects Triangle Pharmanaturals, which bills itself as a consultant and packager of supplement products.

Kratom is a plant supplement sold to treat pain, to help people stop using opioids or as a stimulant. The FDA has been warning against its use in general and later issued a large voluntary recall notice after some samples were found to be contaminated with salmonella and sickened 87 people.
